OutPatient Services for Japanese Health Insurance Card holders

1. The flow of a hospital visit

2. For patients on a first visit

Consultation hours for all departments are 8:30 to 11:00 am. Fill out an Application for Physician Consultation next to the General Information Counter and take the application together with your health insurance certificate, any medical care recipient certificate you have, and a letter of referral from another institution (if you have one) and hand it in to the General Reception Counter Window No. 1. When the reception process is finished, take the file jacket containing your patient registration card and any other items to the department you are visiting.

About the fixed fee (elective consultation fee) charged when visiting the University Hospital for consultation without a referral

This hospital was approved as Special Functioning hospital in November 1994. When patients come here for their first consultation without bringing a letter of referral from another medical institution, you are subject to a fixed fee (elective consultation fee) charged when visiting the University Hospital for consultation without a referral that is in addition to the usual treatment fees, as follows.

  • Medical First visit: 7,700 yen (tax included), return visits: 3,300 yen (tax included)
  • Dental First visit: 5,500 yen (tax included), return visits: 2,090 yen (tax included)

About checking the health insurance certificate, medical care recipient certificate, etc.

We will check your health insurance certificate, medical care recipient certificate, and other such items regardless of whether you are on a first visit or a return visit, so please be sure to present these items at the window on the day of your visit. Please be aware that if we cannot check your health insurance certificate, then you may need to pay the full amount of the health care cost for that day out of your own pocket.

For patients who are recipients of public assistance or who are coming for consultations due to traffic accidents, accidents in the line of duty, industrial accidents, etc.

If you are one of the above, please inform the person at the counter when checking in at reception.

3. For patients on return visits

If you have an appointment, please go directly to the department you are visiting and go through the reception procedure using the automated return visit reception machine. When you have finished the procedure, please place the return visit appointment slip, the patient registration card, and the reception slip in a file jacket and present it at the window.

If you do not have an appointment, but it is within six months since your last consultation in the department you are visiting, then go through the reception procedure using the automated return visit reception machine and present the reception slip and patient registration card to the department reception window.

If it has been six months or more since your last consultation, then go through the return visit reception procedure at General Reception Counter Windows No. 2 to No. 5 (present your health insurance certificate and letter of referral from another institution) and then present the reception slip and patient registration card at the reception window of the department you are visiting.

Important

Starting time and order of consultation are subject to change

Please understand that your appointment time is an indicator of when to come to the hospital, but that the starting time and order of consultations may change due to the medical condition of other patients or other such contingencies.

Initial consultation charge (elective consultation fee) may apply in some cases

Please understand that even after patients have come to this hospital for consultations for two or more times, an initial consultation charge (elective consultation fee) may apply in some cases when the circumstances or substance of consultation result in treatment as a new case (a new diagnosis).

We will check your health insurance certificate, medical care recipient certificate, and other such items regardless of whether you are on a first visit or a return visit, so please be sure to present these items at the window on the day of your visit.

Please be aware that if we cannot check your health insurance certificate, then you may need to pay the full amount of the health care cost for that day out of your own pocket. Please provide immediate notification at the counter if there is any change in the health insurance certificate, such as starting partway through the month.

4. When the consultation has ended

After consultation, tests, and so on have ended, take the file jacket (patient registration card and outpatient consultation management slip) given to you by the physician in charge of the case together with the numbered ticket issued by the numbered ticket dispenser in the waiting room area and present them at General Reception Counter Windows No. 2 to No. 5.
* If this is outside the reception hours for Windows No. 2 to No. 5 (between 5:00 pm and 8:30 am), please present those items to the after-hours reception counter located at the emergency department on the second floor.

If you need to make payment of charges for medical services or if you are being dispensed medicine, you will be called to General Reception Counter Window No. 6. Please wait in the waiting room area until you are called.

<Payment>

Please make payment of your health care cost at General Reception Counter Windows No. 7 to No. 8 or at one of the three automated bill settlement machines. * Debit cards and credit cards can be used to make payment.

<Medicine and Prescriptions>

At this hospital, as a rule we issue prescription slip for use outside the hospital, and we expect patients to obtain medicine at an insurance-approved dispensing pharmacy outside the hospital. Please present your prescription slip for use outside the hospital at the fax corner for prescription slips for use outside the hospital.

5. The Hospital wants you to know:

<About patient registration cards>

  • Patient registration cards are used in common by all our clinical departments. They are for permanent use, so please keep your card carefully and be sure to bring it with you when you visit the hospital.
  • If a patient registration card needs to be reissued because the original was lost or damaged, please buy a card at the convenience store in the hospital, take it to General Reception Counter Windows No. 3 to No. 5, and ask for a reissue.
  • Our patient registration cards are very susceptible to magnetism, so please take care to keep them away from magnetic health aids, televisions, speakers, and other such sources of powerful magnetic fields.

Appointments for a return visit

  • We have taken steps to shorten patient waiting times at this hospital by introducing a reservation system for return visits for outpatient consultation. Please discuss this with the physician in charge during your consultation and decide on the date of your next appointment.
  • If unavoidable circumstances make it necessary for you to change or cancel your return visit appointment, please inform us as far in advance as possible during the hours from 8:30 am to 5:00 pm on weekdays. Telephone the main hospital number: 023-633-1122.
    Please understand that the hospital may be unable to make your next appointment at the time you wish.

Others

  • Please take care of your personal belongings, valuables, and other possessions to ward against theft.
  • The use of mobile telephones can interfere with the operation of medical equipment, so please cooperate by using your mobile telephone only in designated areas. When you bring your mobile telephone into the hospital, please be sure to turn off its power.
  • All hospital building interiors are no smoking areas. We ask everyone to cooperate.

6.Emergency care

  • This hospital is an emergency medical care facility.
  • We accommodate emergency patients on request from secondary emergency medical care facilities and Fire Defense Headquarters. We also accept emergency patients from the public at large.
  • For contact at night and on holidays, the telephone number is 023-635-5100 (after hours reception).
  • Patients who come for emergency medical care at night and on holidays should be aware that they may be charged a special after-hours consultation fee (8,800 yen) if the physician in charge of emergency medical care determines that their case is not necessarily urgent.
